Based on observation, interview, record review, At 11:15am while reviewing staff files 5 out 6 staff members did not have an updated mandated reporter training certificate in their file. The licensee did not comply with the section cited above in which poses a potential health, safety or personal rights risk to persons in care.

Based on observation, interview, record review, At 11:00am while reviewing staff files 3 out 6 staff did not have proof of immunization against pertussis, and measles in the staff’s files. Based on observation, at 9:56am during outdoor inspection LPAs observed broken tricycle seat and playhouse base that can pose a safety risk to children in care. The licensee did not comply with the section cited above in which poses a potential health, safety or personal rights risk to persons in care.

Based on observation and record review, at 10:45am none of the staff members present possesses a current EMSA approved Pediatric CPR/First Aid certifications. Based on observation, At 8:58am 2 additional preschool age children arrived making it 14 children with 1 staff, facility was out of ratio. At 9:03am LPAs took census inside the classroom and there were 27 preschool age children with 2 staff, the facility was out of ratio. the licensee did not comply with the section cited above in which poses an immediate health, safety or personal rights risk to persons in care.

Staff Sonia was not providing supervision at all times while the children were on the playground. The licensee did not comply with the section cited above in which poses an immediate health, safety or personal rights risk to persons in care.
