Based on three out of the five Staff member interviews, preschool and school age children comingle at the end of the day in the school age room or in the preschool playground. This poses a potential risk to the safety of the children in care.

Based on interviews conducted with 3 Staff, and record review, facility was out of ratio nine out of ten days that were reviewed. This poses an immediate safety risk for the children in care.

.3 California Lead Action Level at Child Care Centers (b) Testing results with fractional ppb readings of 5.0 ppb or greater shall be rounded up to the nearest whole number, before comparing to the Action Level. (1) A result with values of 5.5 ppb or greater shall be deemed an Action Level Exceedance. Based on lab report analysis water from outlet C (15.0 parts per billion). Outlet samples were taken on 07/08/2022 indicate levels of lead in exceedance.

(a) The licensee shall ensure that each child is accorded the following personal rights: (3) To be free from corporal or unusual punishment, infliction of pain, humiliation, intimidation, ridicule, coercion, threat, mental abuse or other actions of a punitive nature including but not limited to: interference with functions of daily living including eating, sleeping or toileting; or withholding of shelter, clothing, medication or aids to physical functioning.

(a) There shall be a ratio of one teacher visually observing and supervising no more than 12 children in attendance, except as specified in (b) and (c) below.

Based on staff interviews. A staff member disclosed witnessing Staff 2 (S2) raises S2 voice when children do not listen to S2’s directions. It is loud and can hurt the children and staff's ears when S2 raises S2 voice. This staff member also confirmed witnessing S2 yelled at a child while the child was sitting on a chair and said “get up right now” so another child can sit. This poses an immediate risk to the health of the children in care.
