Based on records review and interview, the licensee did not comply with the section cited above in 2 of 2 licensee's, which poses/posed a potential health, safety or personal rights risk to persons in care. The records review shows the licensees don't have a current mandated reporter in file. The licensee stated both licensees have to renew the mandated reporter.
Based on observation and interview, the licensee did not comply with the section cited above in 5 of 5 children in attendance, which poses/posed a potential health, safety or personal rights risk to persons in care. The licensee's sing in/out sheets don't have a full legal signature for any of the children in attendance. The licensee stated that parents sing at the end of the month.
Based on interview statements, the licensee left chemicals in the daycare area accessible to children in care. This poses an immediate risk to children in care.

This regulation was not met as evidenced by: Based on LPA's observation on 11/14/24 S2 was supervising a few daycare children alone. Records review show S2 was not teacher qualified. This poses a potential risk to the health and safety of children in care.

The director stated the facility has not been tested for lead. The facility has three faucets, one is portable.

The licensee stated that because the facility just recently enrolled children, no disaster drills have been conducted.

(a) Buildings and Grounds. The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors. This regulation was not met as evidenced by: Based on observation and interviews, the director did not ensure a broken mirror was repaired. Tape was not placed on the broken mirror and a repair order was not provided during the visit. This poses a potential risk to the health, safety, and personal rights of children in care.

S3 did not have the required documents in file. Missing were health screening and tb test. This poses a potential risk to the health, safety, and personal rights of children in care.
