Based on observations interview and records review , the licensee did not comply with the section cited above. LPA obserced fire extinguisher was serviced on 08/2024,which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated , fire extinguisher will be serviced and proof will be emailed to LPA by POC date .
Based on observation, interview and record review, the licensee did not comply with the section cited above as licensee and 2 other staff have an expired Mandated Reporter Training which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee and 2 other staff will complete Mandated Reporter Training. Licensee will provide LPA P. Ochoa proof of completed certificates by POC day of 12/06/2024.
