Based on observation, interview, record review, the licensee did not comply with the section cited above in 1 out of 3 staff have not completed a current mandated reporter training, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will ensure that S3 has completed Mandated Reporter training and licensee will send proof od completed certificate to LPA by POC date.
Based on observation, interview, record review, the licensee did not comply with the section cited above in 1 out of 3 staff could not provide proof of measles and TB immunization, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will ensure that S3 obtains proof of measles and TB immunization. Llicensee will send proof to LPA by POC date.
Based on observation, the licensee did not comply with the section cited above in one out of one 2A10BC fire extinguisher indicated fully charged but did not have a service tag which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Per licensee will have 2A10BC fire extinguisher serviced by POC due date and submit picture of service tag/fire extinguisher to LPA.
Based on record review, the licensee did not comply with the section cited above in two out of two staff had an expired CPR/First Aid certificate which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Per licensee staff will complete EMSA approved CPR/First Aid traning and submit copy of certificate to LPA by POC due date.
