Based on observations, records review and interview , the licensee did not comply with the section cited above . Duting the file review 2 out of 8 children`s records were missing medical assesment forms,which poses/posed a potential health, safety or personal rights risk to persons in care.

No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation Child was pulled down a set of steps by other children, which was not witnessed by any staff. Lack of supervision posed an immediate risk to the child in care.

Based on observations, records review and interview, the licensee did not comply with the section cited above. During the file review, LPA obseved staff # 8 was missing proof of immunizations (Tdap, MMR) , which poses/posed a potential health, safety or personal rights risk to persons in care.
Based on records review and interview with Facility Representative ,the licensee did not comply with the section cited above. During the records review , LPA observed 7 out of 7 records were misiing current Mandated Reporter Certificates, existing ones were expired 12/23, which poses/posed a potential health, safety or personal rights risk to persons in care.
