Based on interview and record review, the licensee did not comply with the section cited above in which licensee has not completed the Mandated Reporter training class which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will complete the mandated reporter training class, class is available free online at WWW.MANDATEDREPORTERCA.COM. Once completed licensee must submit a copy of the certificate to the San Jose Regional Office (SJRO) by close of business on August 23, 2023.
Based on record review, the licensee did not comply with the section cited above in which Child 03 is missing LIC 700 and LIC 627 which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will provide LIC 700 and LIC 627 to parents of Child 03 to be completed and signed, licensee will submit a copy of the completed forms to the SJRO by close of business on August 23, 2023.

Resolution: Licensee will conduct a disaster drill and will document and submit the drill to the San Jose Regional Office (SJRO) by close of business (cob) on April 13, 2022.
