Based on record review, the licensee did not comply with the section cited above LPA reviewed 1infant record and Licensee stated that she havent documented the 15 min log because she forgot. this action can poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will send all the 15 min check on the due date by text.

Based on record review, the licensee did not comply with the section cited above. Staff member completed mandated reporter training on 02/27/2020 and has yet to complete renewal mandated reporter training every two years following the date on which he or she completed the initial mandated reporter training which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Staff member will complete mandated reporter training via www.mandatedreporterca.com and submit copy of certificate of completion to Fresno Community Care Licensing by 02/17/2023.

Based on interview and record review, the licensee did not comply with the section cited above in that the last fire drill was conducted on 10/13/2019. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she shall send documentation of a conducted fire drill to the Community Care Licensing (CCL) office. Proof of this correction shall be sent by the given due date.
