Based on interviews, the licensee did not comply with the section cited above. Licensee confirmed Adult 3 and have been present in the facility without a criminal record clearance, which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee provided Adult 3 with a copy of the Live Scan (LIC9163) and instructed Adult 3 to complete the live scan. LPA provided Guardian contact information to Licensee. Licensee stated she will submit proof of live scan to the Fresno Community Licensing (CCL) office by 04/02/2025. Licensee understands that until background clearance is obtained, Adult 3 is not permitted to provide childcare services, or reside within the Family Child Care Home.

Resolution: Licensee stated that they will have parents complete the LIC9224 document and submit it to CCL on or before POC due date.

Based on observation, the licensee did not comply with the section cited above. LPAs observed one infant sleeping in a bouncer placed inside a play yard, covered by a blanket with a propped bottle, which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee agreed that she will watch the informational videos and read information on Safe Sleep available at www.ccld.ca.gov under the section of Safe Sleep. Licensee will provide a written statement in regard to completion of watching the video and best practices moving forward. Proof of written statement is to be submitted to Community Care Licensing (CCL)-Fresno by 4/26/24

LPAs observed spider webs in various play areas, loose wooden fence boards leaned up against the fence to cover broken wooden fence boards of the original fence, dog feces and a hole in the dirt with a large black plastic pipe exposed which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will remove the feces and siderwebs, fill the hole, secure the boards and send verification to community care licensing by POC due date 4/29/24
