Based on records review, the licensee did not comply with the section cited above as child #2 did not have immunization records on file. Furethermore, chidl #1 and child #3's imminuzation records were outdated, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agreed to submit a copy all children's immunization records via email/text by POC deadline 03/09/2026.

Based on observations, statements and records review, the licensee did not comply with the section cited above as adult #1 was determined to live in the home without a criminal record clearance. LPA Herrera observed adult #1 doing her hair and make up and licensee disclosed that adult #1 was residing in the home and identified as her adult daughter, which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Adult #1 left the property during the inspection and licensee stated that she will have her adult daughter complete a livescan and take the necessary steps for her daughter to be cleared. Licensee will submit proof of completion to CCLD via email/text by POC deadline.

Based on observation, the licensee did not comply with the section cited above as the pool was not equipt with a pool alarm, body hook, and a life ring. Furethermore, licensee did not document daily safety inspections, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that she will comply with pool safety regulations and will provide LPA Herrera with a purchase receipt as soon as possible. Licensee agreed to have the required items and document daily safety inspections by POC deadline 03/09/2026. Licensee will submit proof of correction via email/text.

Based on records review, the licensee did not comply with the section cited above as her assistant did not have immunization records on file. Furethermore, the assistant did not have a tuberculosis screening on record, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agreed to submit a copy of her assistant's immunization records and TB screening results via email/text by POC deadline 03/09/2026.
