(a) The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors. This was not met as evidenced by LPA observing the preschool outdoor play yard having insufficient amount of mulch, sand box not being covered, play structure bent, and rats/rat droppings being present in the garbage area.
