Based on observation, interview conducted, and records reviewed, the licensee did not comply with the section cited above as licensee did not ensure mandated reporter training was completed for all staff by the expiration date which poses a potential safety risk to children in care.

Resolution: LPA discussed the mandated reporter training requirement and Licensee was reminded to ensure all staff renew the course every two years. Licensee agreed to provide completed certificate for S2 to LPA by 11/20/2025.

Based on observation, interview conducted, and records reviewed, the licensee did not comply with the section cited above as licensee did not ensure pediatric CPR and first aid certifications for all personnel were approved by EMSA which poses a potential safety risk to children in care.

Resolution: LPA discussed the requirement to renew CPR and first aid certification every two years prior to expiration. LPA also informed Licensee that certifications are to be pediatric and approved by EMSA. Licensee agreed to provide proof of enrollments and/or completed certificates for themselves, S2, and S3 to LPA by 11/20/2025.

Resolution: LPA discussed the requirement to maintain documentation of the required immunizations for all employees and volunteers. Licensee agreed to provide proof of required immunizations for S3 to LPA by 11/20/2025.
