Based on observation and interview, the licensee did not comply with the section cited above in that, Assistant 1 (A1) was supervising twelve children alone for approximately 15 minutes between 1:45PM - 2:00PM, which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Assistant 2 (A2) returned to the facility around 2:00PM and brought the facility back into compliance with staffing ratio.

Based on observation and interview, the licensee did not comply with the section cited above in that, licensee's absence from the facility exceeded 20 percent of the hours that the facility provided care on 7/18/2025, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Assistant 2 (A2)/Facility Representative stated that they will submit a written plan signed by the licensee acknowledging the above regulation and how they plan to follow the regulation going forward. A2 stated that they will email the plan to LPA by due date.

Based on observation, interview, and record review, the licensee did not comply with the section cited above in that, Assistant 1 (A1) and Assistant 2 (A2) did not request and be approved for a transfer to the facility, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: A1 and A2 completed LIC 9182 and emailed it to RO during LPA's visit.

Based on observation and interview, the licensee did not comply with the section cited above. LPA observed S1 and S2 caring for 12 children inside the family child care home. S1 and S2 did not have a current Pediatric CPR/First Aid certification and could not provide proof showing they have completed the training which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee agreed to provide proof showing S1 and S2 has completed Pediatric CPR/First Aid training by POC due date.

Based on observation, interview, and record review, the licensee did not comply with the section cited above. During records review, LPA observed the last fire/disaster drill was conducted on 10/14/2022 which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agreed to submit proof showing a was conducted by POC due date.

Based on observation, interview, and record review, the licensee did not comply with the section cited above. Licensee mandated reporter certification expired 8/19/2021. Assistants present were unable to provide proof of their mandated reporter certification which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agreed to attend training along with S1 and S2 and provide proof of the certification to LPA by POC due date.
