Based on observation, the licensee did not comply with the section cited above. During the visit LPA observed mesh child gate located at the bottom of the stairs to be broken which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she will correct the violation and will send LPA a picture by POC due date.
Based on observation, interview, and record review, the licensee did not comply with the section cited above. At the time of arrival there were 14 children present. One child at 6 years old was present. Licensee stated there were no other school age children present which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she will talk to the families and get back into ratio.
Based on interview, the licensee did not comply with the section cited above. Licensee stated she does not have nap logs for child #10 (C10) which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she will document infant safe sleep and submit documentation to LPA by POC due date.
