Afsaneh has been working at this facility since 2/10/2025. An immediate 500.00 civil penalty was assessed today.
Resolution: The licensee filled out the required paperwork and emailed them to the office to associate Afsaneh Jamalzadeh. LPA will check the system to make sure association is complete.
