(1) A result with values of 5.5 ppb or greater shall be deemed an Action Level Exceedance. This requirement is not met evidenced by record review of the lab report analysis from Outlet A taken on 11/19/2022. Results indicated levels of lead in exceedance. This poses a potential risk to the health and safety of children in care.

Based on observation, interview and record review, the licensee did not comply with the section cited above in 1 out of 3 staff which poses/posed a potential health, safety or personal rights risk to persons in care. Subject staff's Mandated Reporter Training expired on 03/25/2023.

Based on interview, staff did not provide supervision for one child within one minute as children were transitioning inside of the classroom. This poses a potential risk to the safety of children in care.
