Based on interview, the licensee did not comply with the section cited above. Director stated they have not tested their water for lead and that the building was constructed in 2005 which poses/posed a potential health, safety or personal rights risk to persons in care.
Based on observation, interview, and record review, the licensee did not comply with the section cited above. During staff file review, LPA observed staff #2 (S2) and staff #3 (S3) did not have immunization records. All five staff files reviewed did not have influenza records or declaration stating they have denied the flu shot which poses/posed a potential health, safety or personal rights risk to persons in care.
