Based on interview and record review, the licensee did not comply with the section cited above in that, licensee did not maintain a 15 minute Sleep Log for infants under 24 months, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated they will maintain Sleep Logs for infants under 24 months going forward. Licensee stated they will submit copy of Sleep Logs for Child 3 (C3) and Child 4 (C4) for 12/10-12/24 and submit copies of the logs to LPA via email by due date.
Based on interview and record review, the licensee did not comply with the section cited above in that, licensee did not conduct an emergency drill at least once every six months, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated they will conduct an emergency drill and send a copy of the emergency drill log to LPA via email by due date.
Based on observation, interview and record review, the licensee did not comply with the section cited above in 2 out of 2 staff missing immunization, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she will obtain the immunization will send it to LPA by 01/12/2024.
Based on observation, interview, and record review, the licensee did not comply with the section cited above in 2 out of 2 children were misisng the Infant Sleeping Plan, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she will obtained the Infant Sleeping Plan (LIC 9227) from parents and send it to LPA by 12/22/23.
Based on interview with Licensee and record review, at 12:50 p.m., during interview conducted with Licensee, Licensee stated has not conducted a disaster drill in 2 years, and does not document the drills. At 1:30 p.m., during record review, LPA did not observe a disaster drill record on file. The licensee did not comply with the section cited above, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Per Licensee will conduct a disaster drill (Earthquake and Fire) by the due date of 4/26/22. Per Licensee, will submit a copy of the completed drill via email to LPA Gontarek.
