Based on record review, the licensee did not comply with the section cited above in that, C4 and C5 did not have LIC 282 in their records, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated they will submit copies of C4 and C5 LIC 282 to LPA via email by due date.
Based on interview and record review, the licensee did not comply with the section cited above in that, licensee stated that they did not maintain personnel records for A1 and A2, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: LPA provided licensee the Entrance Checklist and reviewed the records needed for personnel files. Licensee stated they will submit copies of personnel files to LPA via email by due date.
Based on record review, the licensee did not comply with the section cited above in that, C1 and C2 did not have immunization records on file for review, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee provided immunization records for C1 and C2 to LPA at the end of the visit.
Based on record review, the licensee did not comply with the section cited above in that, licensee did not maintain PM 286 in all children's files reviewed (C1, C2, C3, C4, C5), which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated that they will complete PM 286 for C1, C2, C3, C4, and C5 and submit copies to LPA via email by due date. LPA advised licensee to review all of the children's records to ensure that each child has a completed PM 286.
Based on interview and record review, the licensee did not comply with the section cited above in that, licensee did not have a current facility roster for review, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated they will submit a copy of current facility roster to LPA via email by due date.
Based on record review, the licensee did not comply with the section cited above in that, licensee, A1, and A2 did not have current mandated reporter training certificates on file, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated that they will submit copies of the mandated reporter training for licensee, A1, and A2 to LPA via email by due date.
Based on observation, the licensee did not comply with the section cited above in that, the child safety gate for the stairs leading to the second floor was left open, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee closed the gate during the inspection. LPA reminded the licensee that the gate needs to remain closed at all times.
Based on observation, the licensee did not comply with the section cited above in that, the kitchen knives were in a drawer which did not have a safety latch leaving the drawer accessible to children in care, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated that they will submit a photo of the safety latch for the drawer containing the kitchen knives to LPA via email by due date. For now, the kitchen knives were moved to an upper cabinet inaccessible to children in care.
Based on observation, the licensee did not comply with the section cited above. LPA observed knives and scissors inside the drawers and detergents under the kitchen sink cabinet that reads " keep out of reach of children.". LPA also observed sharp gardening tools and a propane tank in the out door play area that children utilize for outdoor play which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: LPA observed licensee remove all hazardous items and placed them in areas that are inaccessible to children in care.
Based on observation, interview, and record review, the licensee did not comply with the section cited above. Licensee was unable to provide LPA documentation showing a disaster drill has been conducted within the last six months which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she will conduct and document a fire/disaster drill and submit proof to LPA by POC due date.
Based on observation, interview, and record review, the licensee did not comply with the section cited above. Licensee and assistants present could not provide proof of their mandated reporter certifications which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she and her assistant will attend the training at www.mandatedreporterca.com and submit proof of (AB1207) certifications to LPA by POC due date.
Based on observation, the licensee did not comply with the section cited above. Upon entry, LPA observed the staircase was not barricaded, allowing children to have access to the second floor of the home which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: LPA observed licensee making the second floor inaccessible by means of a safety gate while LPA was present.
