During inspection on 7/1/2024 LPA toured classrooms and observed ripped napping mats in toddler classroom which poses a potential health, safety, or personal rights to persons in care.
Based on record review of staff files by LPA, the facility did not comply with the section cited above in 3 out of 9 staff files reviewed which poses a potential health, safety or personal rights risk to persons in care.

LPA conducted inspections on 7/1/24,8/2/24, and 8/22/24 and there was no director and staff informed LPA there is no director.
Staff interviewed stated they have been left out of ratio themselves and have witnessed other staff being out of ratio which poses a potential health, safety, or personal rights to persons in care.
