Based on observation, interview, and record review, the licensee did not comply with the section cited above. Licensee could not provide immunization records for her assistant. Assistant #1 (A1) is missing their MMR and Tdap which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she will submit proof of A1 immunization records by POC due date.
Based on interview, the licensee did not comply with the section cited above. Licensee could not provide LPA with documentation showing she is conducting infant safe sleep every 15 minutes during nap time which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she will document infant sleep every 15 minutes and send proof to LPA by POC due date.
