Based on observation, at 11:08 a.m., LPA observed a wood board missing under the sink, opening up into the cabinet beneath the sink where cleaning compounds were observed. LPA observed a latch on the cabinet doors. Pictures were taken. Licensee immediately removed cleaning compounds and stored in garage. The licensee did not comply with the section cited above, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Per licensee, will fix cabinet, and install wood piece by the due date of 2/11/22.
Based on interview and record review, at 11:11 a.m., during interview with licensee, licensee stated has conducted fire drills every now and then, but has never documented the drills. During record review, LPA did not observe a disaster drill record. the licensee did not comply with the section cited above, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Per Licensee, will conduct a fire and earthquake drill by the due date of 2/11/22. Per Licensee, will document drill on drill log, and email drill to LPA Gontarek.
Based on observation, at 10:43 a.m., LPA observed (2) play pens and (2) cribs with loose fitting, over-sized sheets. The sheets can easily become dislodged. Pictures were taken. The licensee did not comply with the section cited above, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Per licensee, will purchase 4 tight-fitting sheets appropriate to the mattress sizes by the due date of 2/4/22. Per licensee, will send a picture of the tight-fitting sheets via email.
Based on record review, the licensee did not comply with the section cited above in that at 12:30 p.m., during record review, LPA observed Pediatric CPR and First Aid to be expired for the licensee, expiring on 1/22/2022. which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Per Licensee, will complete Pediatric CPR and First Aid course, and submit proof of completion certificate by the due date of 2/21/22.
