Based on interview and record review, the licensee did not comply with the section cited above in that, licensee did not ensure that LIC 9227 was completed for Child 4 (C4) which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated they will submit a copy of LIC 9227 for C4 to LPA via email by due date.
Based on interview and record review, the licensee did not comply with the section cited above in that, licensee did not complete and maintain a Sleep Log for infants 0-24 months, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated they will provide LPA a copy of the Sleep Logs for Child 2 (C2) and Child 4 (C4) via email by due date.
