Based on observation and record review, the licensee did not comply with the section cited above in one count out of two counts of adults' files that Adult # 2 missing TB clearance on file which poses a potential health, and safety risk to persons in care.

Resolution: The licensee agreed to send the correction to LPA's email address by the due date of 3/12/2025

Based on observation and record review, the licensee did not comply with the section cited above in one count out of two counts of adults' files that Adult # 2 missing MMR and Dtap immunization on file which poses a potential health, and safety risk to persons in care.

Based on observation, interview and record review, the licensee did not comply with the section cited above. During record review, Licensee was unable to provide proof showing a fire drill and disaster drills are being conducted at least once every six months which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated a fire/disaster drill will be conducted and documented by POC due date. Proof will be sent to LPA

Based on observation, interview, and record review, the licensee did not comply with the section cited above. Licensee and assistant was unable to provide proof of their mandated reporter certifications which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated proof will be sent to LPA by POC due date.

Based on observation, interview, and record review, the licensee did not comply with the section cited above. At the time of arrival, LPA observed stairs leading to the second floor of the home was not fenced or barricaded which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: LPA observed licensee place a gate at the bottom of the staircase during visit.

Based on observation, interview, and record review, the licensee did not comply with the section cited above. During children file reviews, LPA observed 4 out of 7 children were missing immunization records which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated immunization for 4 children will be submitted by POC due date.

Based on observation, interview, and record review, the licensee did not comply with the section cited above. During file reviews, LPA observed assistant (S1) did not have copies of their immunization records inside the file. S1 could not provide proof of immunization records at the time of visit which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated proof of immunization records for assistant will be submitted by POC due date.

Based on observation, interview, and record review, the licensee did not comply with the section cited above. LPA observed Suraya Mir Khil inside the living room are of the home providing care and interacting with day care children present. Licensee stated Suraya Mir Khil has been working at the day care since May 2023 which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will have Suraya Mir Khil submit fingerprint documents.
