Based on observation, interview and record review, the licensee did not comply with the section cited above in 2 out of 3 alcohol wipes were accesible to children in care which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated they will remove the alcohol wipes (that say keep out of reach of children) from the day care area. LPA Castro observed Licensee remove the alcohol wipe containers & place them out of reach of children.

Based on observation, interview and record review the licensee did not comply with the section cited above in 2 out of 2 staff files did not have a valid CPR/1st aid certification which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that she will email LPA Castro verification that the CPR/1st aid has been renewed (expired 02/2025).

Based on observation, interview and record review, the licensee did not comply with the section cited above in 2 out of 14 children did not fit the criteria for having additional children (age/enrollment) in care which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that she will submit documentation that the additional child who is enrolled in TK/elementary school. She will also disenroll the child who is not 6 years of age or older.

Based on interview and records review, the licensee did not comply with the section cited above in one out of one assistant preset at the facility, which poses/posed a potential health, safety or personal rights risk to persons in care. The assistant present did not have the required immunization in file.

Resolution: The licensee stated she will obtain a copy of the immunizations and email them to the LPA to the email provided by the due date.
