Based on observatio and, interview, the licensee did not comply with the section cited above. While touring the facility at 11:20 AM, Director could not locate a carbon monoxide alarm. Director went to the location where the carbon monoxide alarm was placed and noticed the carbon monoxide alarm was missing which poses/posed a potential health, safety or personal rights risk to persons in care.

(a) The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors. (1) The licensee shall take measures to keep the center free of flies, other insects, and rodents.

.3 (b) Testing results with fractional ppb readings of 0.5 ppb or greater shall be rounded up to the nearest whole number, before comparing to the Action Level. (1) A result with values of 5.5 ppb or greater shall be deemed an Action Level Exceedance. This requirement is not met evidenced by record review of the lab report analysis from Outlet A taken on 02/07/2023. Results indicated levels of lead in exceedance in outlet J. This poses a potential risk to the health and safety of children in care.
