Licensee and three assistants could not provide LPA with their current mandated reporter certification during the inspection which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on interview and record review, the licensee did not comply with the section cited above. The licensee EMSA approved Pediatric CPR/First Aid certification expired 3/24/2024. Three assistants present did not have an approved Pediatric CPR/First Aid certification which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee to submit proof showing she has a current Pediatric CPR/First Aid certification by POC due date.
Based on interview and record review, the licensee did not comply with the section cited above. Assistant #2 (A2) assistant #3 (A3) could not show proof of immunization against pertussis and measles. A3 was also missing their TB Clearance which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee to submit proof of immunization records to LPA by POC due date.
