Based on observation and record review, the licensee did not comply with the section cited above in one count out of one file missing CPR and 1st aid for adult # 1 which poses a potential health, and safety's risk to persons in care.

Resolution: The licensee agreed to obtain CPR and 1st aide training completed and send it to LPA's email address by the due date of 10/30/2024.

Based on observation and record review, the licensee did not comply with the section cited above in one count out of two files, missing MMR and Flu shot for adult # 2 which poses a potential health, and safety's risk to persons in care. Flu shot may be declined by a written statement.

Resolution: The licensee agreed to obtain MMR vaccine for Adult # 2 and send it to LPA's email address by the due date of 10/30/2024.

Based on observation and record review, the licensee did not comply with the section cited above in four counts out of twelve files, missing immunization for Child # 3, 4, 9, and 11 which poses a potential health, and safety's risk to persons in care.

Resolution: The licensee agreed to obtain immunization for the above children and send it to LPA's email address by the due date of 10/30/2024.

Based on observation and record review, the licensee did not comply with the section cited above in four counts out of four files, missing log for every 15 minutes check on children under 24 months old for Child # 2, 9, 10, and 12 which poses a potential health, and safety's risk to persons in care.

Resolution: The licensee agreed to complete the log for every 15 minutes check on the above children and send it to LPA's email address by the due date of 10/30/2024.
