Based on record review, the licensee did not comply with the section cited above in 5 out of 7 children's files reviewed were incomplete missing LIC627 Concent for Emergency Treatment and C#4 with incomplete emergency contact information which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: LIcensee stated they will provide parents with the required LIC627 Concent for Emergency Treatment and provide C#4 with a new LIC700 Identification and Emergency to be completed. Licensee stated they will email required documents to LPA Lowe no later then end of business on 4/29/2022.
