Based on interview and record review, the licensee did not comply with the section cited above in that both licensees do not have record on file of measles and pertussis immunizations which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to submit proof of immunizations for measles and pertussis both licensees by due date 12/30/2022.

Resolution: Licensee agrees to purchase a crib or play yard for infant to sleep in by due date 12/02/2022 , and will submit picture of the product to the Department.
