Facility included in their plan of operation the following information related to COVID-19 preparedness and emergency plan “…If you travel for medical or dental appointments, you may be exempt to quarantine if you provide a medical note. This requirement was not being met as evidenced: On 06/2021 and/or 07/2021, facility failed to adhere to this plan on file by not allowing child(ren) to attend daycare after quarantine was served;although, parent/legal guardian provided a doctor’s note to exempt the child. “This poses a potential risk to the Health, Safety, and Personal Rights to the children in care".

All individuals subject to a criminal record review pursuant to Health and Safety Code Section 1596.871 shall prior to working, residing or volunteering in a licensed facility: (2) Request a transfer of a criminal record clearance as specified in Section 101170(f) This requirement was not being met as evidence by: Ms. Romanello was not listed to the facility license, even though she has been working at the facility since March 2019. LPA verified Criminal Record Clearance with the Riverside Child Care Regional Office and discovered the the staff was cleared and associated to all other sister locations. This poses a potential risk to the Health and Safety of the children in care.
