Based on observation, the licensee did not comply with the section cited above in that 2 of 2 infants were sleeping in day care room with door closed which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated they will review and read infant safe sleep regulations. Licensee will send email to LPA confirming they have read and understand all safe sleep regulations. The plan of correction is due 12/20/2024.

Based on interivew and record review, the licensee did not comply with the section cited above in that Licensee did not have complete sleep logs for C4 and C5 which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated they will purchase clip board or folder and place next to play pen for each infant in care to ensure they remember to document the 15 minute check. Licensee will submit a weeks worth of 15 minute check to LPA via email by 12/20/2024.
