Based on record review, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. When LPA arrived, both C1 and C2 were asleep, however, their sleep log was not complete for today 2/12/26.

Resolution: Licensee will send a copy to CCL of C1 and C2's sleep log by the due date.

Based on record review, the licensee did not comply with the section cited above in that Licensee's mandated reporter training was not available upon request whiich poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agreed to submit proof of completion to CCL by/or before the due date.

Based on record review, the licensee did not comply with the section cited above in assistants do not have current mandated reporter training, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit copies of mandated reporter training certificates to department by 2/16/2024.

Based on observation, interview and record review, the licensee did not comply with the section cited above in 9 children present in care and Licensee had no assistant present on this date which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to submit a written statement of understanding and plan on how she will ensure compliance in the future to ensure she is not out of complaince.
