ROBAYO-LLANOS FAMILY CHILD CARE

Based on observation & interview, the licensee did not comply with the section cited above in which poses a potential health, safety or personal rights risk to persons in care. Staff 1 reported that the Licensees were in Los Angeles today. LPA observed that Licensee's did not arrive to the facility in the duration of the inspection. S1 stated that S1 and S2 assisted in providing care yesterday and today.

Resolution: Licensee's will provide CCL with a written statement of their understanding of Title 22 regulation 102417(a) by the due date.

Based on record review, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. Staff 2 (S2) does not have a fingerprint clearance that is associated with the facility.

Resolution: Licensees will provide CCL with a request for criminal record transfer (LIC 9182), or Licensee will stop using S2 as an assistant.

Based on record review, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. Staff 1 (S1) only has a criminal record clearance in their file. Staff 2 (S2) has no file in the facility.

Resolution: Licensees will provide CCL with proof that they created a file for S1 and S2. S1 and S2's need the following items in their file: LIC 9108, LIC 9052, TB, immunizations, CPR/1st Aid, & mandated reporter certificate.

Based on interview and records review, the licensee did not comply with the section cited above in that licensees did not have the mandated repporter training (child care provider and general training) completed, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee Nubia Robayo agrees to submit proof of completed training (general training and child care provider training) to the Department by due date 04/28/2023.
