Based on observation and record review, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. 2 out of 2 infants had incomplete sleep logs. LPA arrived when the children were napping and the logs were not completed.

Resolution: Licensee will complete sleep logs for C1 and C4 and submit a copy to CCL by the due date.

Based on observation, the licensee did not comply with the section cited above as Child 2 had a balnket in the crib which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee removed blanket during the inspection.

Based on record review, the licensee did not comply with the section cited above in 5 out of 5 children did not have PM 286 completed which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit forms to CCL by POC due date.

Based on observation, the licensee did not comply with the section cited above in that the backyard has worn toys and miscellaneous items, such as stacked trash bags which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will remove/dispose of worn toys and place potential hazardous items in off-limit area in 30 days.

Based on record review, the licensee did not comply with the section cited above in that none of the immunization records have been transferred to the PM286 and two children did not have the immunizations in their records, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit copies of completed PM286 by 2/29/2024

Based on record review, the licensee did not comply with the section cited above in that form was not in the file for C6 which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit copy of LIC9227 to the department by 2/29/2024.

Based on record review, the licensee did not comply with the section cited above in 6 out of 6 records, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit copies of LIC9150 for all 6 children files to the department by 2/29/2024.

Based on observation, the licensee did not comply with the section cited above in that an area of the backyard has several items including but not limited to a spray can, unrolled garden hose, items stacked haphazardly, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit a picture showing that potential hazards have been made inaccessible or removed. To be submitted by 2/29/2024.
