Per staff’s own admissions, facility failed to provide adequate supervision for a child(ren) in care. Staff were not aware of the incident until few minutes later another staff from a different classroom saw the child unattended on the playground. Leaving child(ren) unattended presented an immediate risks to the child(ren)'s health, safety, and well- being. The absence of supervision is a zero-tolerance violation.
Per staff own admission, facility failed to report the incident that a child was left unattended on the playground. No additional documentation was provided as confirmation of UIR submission to CCL during the course of the investigation process. This is a potential risk to the health and safety of children in care.

This requirement was not being met as evidenced by the record of immunization against MMR was not available for review in staff (#1) file. This poses a potential risk to the Health, Safety, and personal rights of the children in care.
