Based on record review, the licensee did not comply with the section cited above iwhich poses a potential health, safety or personal rights risk to persons in care. Upon LPA's arrival, Staff 1, who does not have a CPR/1st Aid certificate, was alone with the children while Licensee picked up school-aged children.

Resolution: Licensee will send CCL a copy of Staff 1's CPR/1st Aid Training by the due date.

Based on record review, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. Child 1 (C1) had missing emergency contact information on her LIC 700 and Child 6 (C6) had a missing consent for emergency medical treatment (LIC 627).

Resolution: Licensee will provide CCL with a copy of C1's complete LIC 700 and C6's LIC 627 by the due date.

Based on record review, the licensee did not comply with the section cited above iwhich poses a potential health, safety or personal rights risk to persons in care. Staff 1 is missing proof of MMR vaccine.

Resolution: Licensee will provide a copy to CCL of Staff 1's MMR vaccine by the due date.

Based on record review, the licensee did not comply with the section cited above in that C2 and C5 are not on the roster, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit a copy of completed roster to department by 2/29/2024.

Based on record review, the licensee did not comply with the section cited above in 3 out of 5 records, which poses a potential health, safety or personal rights risk to persons in care.

Based on record review, the licensee did not comply with the section cited above in that Staff 1 does not have copies of immunizations and TB, which poses a potential health, safety or personal rights risk to persons in care.

Based on observation, the licensee did not comply with the section cited above in that an infant who had just awakened from a nap was in a playpen with blankets, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit a statement regarding infant safe sleep regulations to department by 2/29/2024.

Based on record review, the licensee did not comply with the section cited above in that a form was not present for C2 and forms for C3 and C4 were missing signaures, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Since C3 and C5 are not over 12 months old, only a completed form for C2 will be submitted to the department by 2/29/2024.

Based on record review, the licensee did not comply with the section cited above in that a complete sleep log was not present for C2, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: LPA provided a sample sleep log to licensee with requirements of: Name, Date and 15 minute checks per day. Licensee will submit 3 days worth of sleep log to department by 2/29/2024.

Based on record review, the licensee did not comply with the section cited above in that C2 does not have immunization record or completed PM286, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit a copy of PM286 to department by 2/29/2024.

Based on observation, interview, record review, the licensee did not comply with the section cited above in Child #4-8 were not listed on the facility roster, lic9040 which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to update and add Child #4-8 to the Roster, LIC9040 and send a copy to the Department as proof of correction by 6/30/23.

Based on observation, the licensee did not comply with the section cited above in that several cleaning products were observed under the kitchen sink/counter containing warning label to "keep out of reach of children" the cabinet was unlocked which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: The assistant locked and removed all items during visit.

Based on observation, interview, record review, the licensee did not comply with the section cited above in Staff #2 is not cleared and associated to the facility which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to not have Staff #2, present at the facility until staff has been cleared and has obtained clearance and associated to this facility. Submit proof of association to the Dept. by 6/17/2023.

Based on record review, the licensee did not comply with the section cited above in that a disaster drills have not been conducted since 4/22/22, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to conduct drills with children and submit proof of completion by 6/30/2023.

Based on observation, interview, record review, the licensee did not comply with the section cited above in Staff #2 has not completed the required training, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to have Staff #2 completed Mandated Reporter Training-General and Child Provider Training and submit proof to the Department by 6/30/2023.

Based on observation, interview, record review, the licensee did not comply with the section cited above in that Staff #2 did not have any of the required immunization on file which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to obtain and maintain Staff #2 immunization records and submit a copy to the dept. by 6/30/23.

Based on observation, interview, record review, the licensee did not comply with the section cited above in Staff 1-2 did not have proof of first aid/cpr certificates which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to submit copies of Staff 1-2 First aid/CPR cards approved by EMSA with stickers if not from the American Heart Association or American Red Cross to the dept by 6/30/23.

Based on observation, interview, record review, the licensee did not comply with the section cited above in that Staff #2 had no file with any of the requirements which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agree to obtained and maintain all requirements for Staff #2 in file. Submit a copy to the department by 6/30/2023.

Based on observation, interview, record review, the licensee did not comply with the section cited above in Staff #2 did not have proof of the required immunizations on file, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agree to obtained and maintain all staff immunizaiton on file and submit a copy to the Department by 6/30/2023.

Based on observation, interview, record review, the licensee did not comply with the section cited above in Child #4-8 did no have a file with all of the required records which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to obtain and submit Child #4-8 files to the department as proof of correction by 6/30/23.

Based on record review and observation, the licensee did not comply with the section cited above in that 3 out of 6 children onsite during inspection were not added to the roster, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit a copy of the completed roster to LPA by 6/30/22.

Based on record review, the licensee did not comply with the section cited above in that 3 out of 10 enrolled children were missing LIC700, along with all required licensing forms, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will complete the children's files and submit proof to LPA by 6/30/22.

Based on record review, the licensee did not comply with the section cited above in that licensee and 3 out of 4 assistants did not have current mandated reporter training completed, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit copies of all mandated reporter training certificates to LPA by 6/30/22.

Based on observation, the licensee did not comply with the section cited above in that the outdoor play structure has support beams which are broken and splintering, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to remove the play structure by 6/30/22. Children will not be allowed to play on the structure pending removal.
