Upon LPA's arrival, Staff 1, who does not have a CPR/1st Aid certificate, was alone with the children while Licensee picked up school-aged children.
Resolution: Licensee will send CCL a copy of Staff 1's CPR/1st Aid Training by the due date.
Based on record review, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. Child 1 (C1) had missing emergency contact information on her LIC 700 and Child 6 (C6) had a missing consent for emergency medical treatment (LIC 627).

Resolution: Licensee agrees to obtain and maintain Staff #2 immunization records and submit a copy to the dept. by 6/30/23.
Based on observation, interview, record review, the licensee did not comply with the section cited above in Staff 1-2 did not have proof of first aid/cpr certificates which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to submit copies of Staff 1-2 First aid/CPR cards approved by EMSA with stickers if not from the American Heart Association or American Red Cross to the dept by 6/30/23.
