Based on record review, the licensee did not comply with the section cited above in three (3) of three (3) instances. C2, C4, and C6 did not have sleep logs maintained. This poses a potential health, safety or personal rights risk to children in care.
Resolution: Licensee will begin documenting and maintaing a sleep log for C2, C4, and C6 unitl they reach 24 months of age. Licensee will submit proof of such logs to the Department by 6/30/2025.

Resolution: Licensee will ensure an Individual Infant Sleep Plan is completed for C2 and maintained in their file. Licensee wil submit proof of the Plan to the Department by 6/30/2025.
