LPA met with Site Supervisor Laura Diaz Belman. LPA toured the facility, took census and discussed the reason for the case management visit. It was reported by confidential witnesses that the facility had not provided information to parents related to the Type A citation received by the facility on 12/17/24. Site Supervisor stated that they had parents sign LIC9224, regarding the outcome of LPA's visit on 12/17/24; however, they did not include copy of the report. Deficiency was cited. On today's visit, 12/19/2024, LPA observed that the report that was left on 12/17/24 was not posted with the other postings for parents to review. A deficiency and an immediate $100 civil penalty was issued as a result. An exit interview was held with Site Supervisor Laura Diaz Belman and a copy of this report, along with copies of the LIC809D (deficiency page), LIC421CC (Civil Penalty page), and Appeal Rights were discussed and provided. A Notice of Site visit was issued and must remain posted for 30 days. This report shall be public record for three years.
