Based on observation, the licensee did not comply with the section cited above in which poses an immediate health, safety or personal rights risk to persons in care. During the time of visit, there was no assistant present, therefore, staff ratio and capacity reverts to Small License staffing and ratio. At 12:30 PM, LPA observed that the Licensee had a total of 7 children; 2 children were infants and the remaining children were under 6 years of age and none are enrolled in school
Resolution: At 2:00 PM, a child was picked up by the parent bring the Licensee back into Title 22 Staffing Ratio and capactiy. Licensee will provide a written statement to CCL by due date of her understanding of Staffing ratio/capacity and a plan to remain in ratio.
Based on record review, the licensee did not comply with the section cited above in 2 out of 2 children had a sleeping log that was incomplete which poses a potential health, safety or personal rights risk to persons in care. Per regulation : (j) The provider shall supervise infants while they are sleeping and adhere to the following requirements: (1)The provider shall physically check on the infant every 15 minutes.
Resolution: Licensee will submit a copy ti CCL of the 15 mintue log for child 1 and 2 by due date.
Based on record review, the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care. Staff 1 does not have immunizations on file.
Resolution: Licensee will submit a copy of immunizations to CCL by due date.
Based on record reviews, the licensee did not comply with the section cited above, mandated reporter training had expired which posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will email LPA completed training certificate.
Based on record review, the licensee did not comply with the section cited above in that 2 out of 2 files did not have completed sleep logs which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will submit completed sleep logs (5 days worth) for C1 and C2.
