(a) The licensee shall provide care and supervision as necessary to meet the children's needs.(1) No child(ren) shall be left without the supervision of a teacher at any time... (1). Supervision shall include visual observation. This regulation was not met as evidenced by: Based on interview the licensee Children were left without supervsion of a teacher on 2 of 2 occasions ... which poses a potential Health, Safety, Personal Rights risk to persons in care.
.3(c) Teacher-Child Ratio (c) Child...programs funded...Department of Education...under Title 5...Title 5 staffing ratios shall apply in such centers. This requirement was not met as evidence by: Based on interview and record review, Principal did not ensure Title Five ratio was met. Classroom had 30 children supervised by one qualified teacher and four assistants, putting the classroom out of ratio by six children. This poses a potential health and safety risk to children in care.
Based on observation, the licensee did not comply with the section cited above. Two carpets were observed to be unsanitary, which poses/posed a potential health, safety or personal rights risk to persons in care.
(a) The licensee shall ensure that each child is accorded the following personal rights: (3) To be free from corporal or unusual punishment, infliction of pain, humiliation, intimidation, ridicule, coercion, threat, mental abuse or other actions of a punitive nature including but not limited to: interference with functions of daily living including eating, sleeping or toileting; or withholding of shelter, clothing, medication or aids to physical functioning.
