Based on record review, the licensee did not comply with the section cited above last disaster drill logged in April of 2023 which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will conduct and log disaster drills and submit log to LPA via email as proof of correction by agreed upon date.
Based on record review, the licensee did not comply with the section cited above in 1 out of 1 infants enrolled did not have required sleeping log which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will create and keep a sleeping log for all infants enrolled. Licensee will log sleeping for 2 weeks and sleep log to LPA as proof of completion by agreed upon date.
Based on record review, the licensee did not comply with the section cited above in 1 out of 2 staff did not have required Mandated reporter training on file which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will have assistant complete Mandated reporter training and submit certificate as proof of completion to LPA via email by agreed upon date.
Based on record review staff #1 and staff #2 did not have proof of immunizations (MMR,Tdap,Influenza & TB which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will make provide proof of correction by POC due date to LPA.
Based on record review staff #2 fingerprint clearance is in process in the Care Provider Management Branch system, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated that she will make sure to call the department to confirm fingerprints have cleared and associated to her facility.
Based on observation there were at approximately 2:45 PM there were 9 children without an assistant present. LPA observed C#5 and C#9 were picked up at approximately 2:55 PM., the licensee did not comply with the section cited above which poses an immediate health, safety or personal rights risk to persons in care.
Based on record review staff #1 and staff #2 did not have proof of mandated reporter training which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated that she and staff #2 will take the training. LPA provided Mandated Training website via text to the licensee.
