Based on observation, the licensee did not comply with the section cited above. While touring the home at 12:24 p.m. LPA observed facial products in the designated childrens restroom inside an unlocked cabinet. On the back of the product it reads "keep out of reach of children which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: LPA observed licensee remove the products during the inspection.
Based on observation, the licensee did not comply with the section cited above. LPA observed an infant child (C1) alone inside the master bedroom with the doors closed inside a crib with a blanket covering the face of the child which poses an immediate health, safety or personal rights risk to persons in care which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: LPA observed blanket removed from inside the crib and door remained open during inspection.
Based on observation and interview, the licensee did not comply with the section cited above. LPA observed an uncharged 2A-10BC fire extinguisher inside the kitchen. Licensee was reminded The pressure gauge on the 2A-10BC fire extinguisher(s) need to indicate fully charged which poses/posed a potential health, safety or personal rights risk to persons in care. Licensee stated there are no other fire extinguishers inside the home.
Resolution: Licensee agreed to purchase a new fire extinguissher or service the existing fire extinguisher and send proof to LPA by POC due date.
Based on observation, interview, record review, the licensee did not comply with the section cited above. Licensee was unable to provide documentation showing a fire/disaster drill is being conducted at least once every six months which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agreed to conduct and document a fire/disaster drill and submit proff to LPA by POC due date.
Based on observation, the licensee did not comply with the section cited above. LPA observed C1 inside the crib with a blanket which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: LPA observed licensee remove the blanket during the inspection.
Based on observation, interview, and record review, the licensee did not comply with the section cited above. During file reviews LPA observed licensee was not documenting infant safe sleep for September 15, 2023 which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agreed to document infant safe sleep and submit proof to LPA by POC due date.

Resolution: LPA observed licensee open the doors to the master bedroom during the inspection.
Based on observation, interview, and record review, the licensee did not comply with the section cited above. During file review, LPA observed licensee and two staff did not have a current mandated reporter certification which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agreed to to take the mandated reporter training and have both staff condcut the training. Licensee agreed to submit proof to LPA by POC due date.
Based on observation, interview, and record review, the licensee did not comply with the section cited above. Licensee was unable to provide a file for staff #2 (S2) during the inspection which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agreed to create a file for S2 and submit proof to LPA by POC due date.
