Based on observation, interview, and record review. The licensee did not comply with the section cited above in S1 and S2 Mandated Report Training Expired on 9/18/2023. The licensee admitted that S1 and S2 did not have the current mandated reporter training certificate in file, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee states she will submit S1 and S 2 updated/current Mandated Reporter training certificate to LPA by the due date.

Based on observation, interview, record review. , the licensee did not comply with the section cited above because there is no documented log of 15 minute checks to infant C1 and C3 in care, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will create 15 minute check log for infants C1 and C3 in care. Licensee will submit one week of completed logs to LPA via email by POC date.

Based on LPA's observation, the licensee did not comply with the section cited above in 1 out 1 infant in a play yard with a small self soothing blanket which poses an immediate safety risk to persons in care.

Resolution: Licensee will not allow infant to sleep with the small blanket inside the play yard. Licensee will speak to parents to notify them of the change. Licensee will submit a declaration to LPA by the POC date.

Based on LPA's observation, the licensee did not comply with the section cited above in allowing children to utilize 2 bedrooms not listed on facility sketch which poses a potential safety risk to persons in care.

Resolution: Licensee will email new floor plan to LPA by POC date. Licensee will communicate to Licensing office if any changes are made to the physical plant of the facility.

Based on record review, the licensee did not comply with the section cited above because there is no documented log of 15 minute checks to infant in care which poses a potential health and safety risk to persons in care.
