Based on record review, the licensee did not comply with the section cited above. Licensee was not able to provide documentation of current Mandated Reporter Training Certificate. This poses a potential health, safety or personal rights risk to persons in care.

Based on observation, interview and record review, the licensee did not comply with the section cited above for one staff that was left alone with children. which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee stated they will submit proof of CPR enrollment to the department via email olivia.meza@dss.ca.gov by 3/7/2025.

Based on interview and record review, the licensee did not comply with the section cited above in 4 out of 4 infant files which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated they will submit a 15 minute sleep check for all infants and submit proof to the department via email olivia.meza@dss.ca.gov by 3/14/2025.

Based on interview and record review, the licensee did not comply with the section cited above and does not have a current children roster on file for review. which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that they will submit child roster to the department via emai olivia.meza@dss.ca.gov l by 3/14/2025.

Based on interview and record review, the licensee did not comply with the section cited above 4 (four) out of 4 (four) children files which poses a potential health, safety or personal rights risk to persons in care.

Resolution: This will be amended. There are no infants from 0-12 months.

Based on LPA's observation, the licensee did not comply with the section cited above, infant napping in Room 2 was laying next to a large blacket in their crib which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee states she will make parents aware that blankets are not allowed in the crib with the child. Licensee will work with family to help child transition. LPA will provide Licensee with Safe Sleep regulations and resources. Licensee will provide information to families and request signature upon reciept. Licensee will email copy of signature page to LPA by POC date.

Based on LPA's record review, the licensee did not comply with the section cited above, no sleep logs were found in infants under 24 months files which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will create a 15 minute check log. Licensee will email LPA one weeks worth of logs for each child under 24 months by POC date.
