It was reported by Marsha Linquist, Center Manager, the unusual incident occurred on 09/12/2024, when Child #1 (C1) was left unattended in the playground for approximately 10 minutes after the children were transitioned into the classrooms. Center Manager stated the following: Staff #1 (S1) and Staff #2 (S2) were with 16 children on the playground. They transitioned from outside to inside. C1 was not in their group. C1 was hiding behind the play structure. Staff #3 (S3) came to the classroom to help with children's lunch, saw through the glass door that C1 was walking from behind the play structure to the classroom. C1 was left unattended for 10 minutes on the playground. During today's inspection, a tour of the facility was conducted. There were 5 children supervised by 2 staff members in classroom Preschool Three, 15 children supervised by 2 staff members in classroom Preschool 2 and 4 children supervised by 2 staff members on the playground. Facility was operating within compliance with capacity and staff to child ratio. LPA Tran met with Site Supervisor Estefany Lopez and informed Mrs. Lopez purpose of the visit. A review of staff records on this date indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances. During today's inspection staff members were interviewed. During the interviews, all 3 interviewed staff confirmed that C1 was left unattended in the playground for approximately 10 minutes. S1 admitted that S1 and S2 did not conduct name-to-face to account for each child who was outside during the transition. Site Supervisor Lopez stated facility had conducted meetings to address concerns individually with involved staff members, and a retraining for all staff members in regarding to Supervision Best Practices and Name-to-Face procedure. The deficiency is being cited on the attached 809D. Due to the Type A violation the licensee shall post and provide copies of the report to parents/guardians of the children in care at the facility by the next business day and shall provide to the parents/guardians of children newly enrolled at the facility during the next 12 months. In addition, the licensee shall immediately post upon receipt the Proof of Correction for 30 consecutive days. The licensee is to keep Acknowledgement Receipt (LIC9224) signed by parents in each child’s file. Exit interview was conducted. The Notice of Site Visit was posted. Site Supervisor was advised the Notice of Site Visit must be posted for 30 days or 100$ Civil Penalty will be assessed. Appeal rights provided and explained. The facility representative was provided a copy of their appeal rights (LIC 9058) and their signature on this form acknowledges receipt of these rights.
