Infant Safe Sleep (j)(2)(D) Documentation shall be maintained in the infant’s file and be available to the Department for review. Documentation shall include the following: a. Date. b.Infant’s name. c.Time of each 15-minute check. Based on record review and interview, the licensee did not comply with the section cited above in four out of four child files which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will provide documentation of 15 minute sleep checks for four out of four infants in care to the department via email by the due date of 10/24/2025. olivia.meza@dss.ca.gov

Based on observation, interview, record review. The licensee did not comply with the section cited above S1 and S2 admitted that they did not have the current mandated reporter training certificate, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee states she will submit the updated/current Mandated Reporter training certificate to LPA by the due date.

Based on observation, interview, record review, The licensee did not comply with the section cited above S2 did not have immunized against influenza, pertussis, and measles, in the files, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee stated she will submit to LPA , the proof of immunization (influenza, pertussis, and measle) record for S2 before the due date.

Based on record review, the licensee did not comply with the section cited above because they could not locate their Mandated Reporter certificate which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will complete training and email Mandated Reporter training by POC date.
