Based on LPA's record review of staff files, the licensee did not comply with the section cited above in 1 out of 4 staff members did not have a mandated report training certificate of proof training was completed which poses a potential health, safety or personal rights risk to persons in care.

.3 (b) Testing results with fractional ppb readings of 0.5 ppb or greater shall be rounded up...before comparing to the Action Level(1)A result with values of 5.5 ppb or greater shall be deemed an Action Level Exceedance. This requirement is not met evidenced by record review of the lab report analysis from outlet Q, S, T, L, H, I, E, F, C, D, B, and G taken on 10/26/2022. Results indicated levels of lead in exceedance. This poses a potential risk to the health and safety of children in care.

Buildings and Grounds (a) The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors. This requirement is not met evidenced by: Based on facility Lead sampling tests it was discovered that drinking fountain/outlet in Room 2, 5, 6, 7,9, 10, 12 had high level of lead. This poses a potential risk to the health of children in care.

Based on LPA record review, the licensee did not comply with the section cited above in 5 out of 5 staff member's files CPR /First Aid certifications expired 04/2021 which poses an immediate health and safety risk to persons in care.
