Based on record review, the licensee did not comply with the section cited above: C1, C2, C3, C4, C8 are missing MMR vaccine update in files, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will provide updated MMR vaccination proof for all children that were missing the proof in thier files. Licensee will submit the proof by Plan of Correction Date: April 1, 2024.
Based on observation, the licensee did not comply with the section cited above. LPA observed the inside of the home was remodeled including kitchen wall was taken down, the bathroom for children was relocated to another location, the washer and dryer was moved to where the original bathroom for children was. Licensee stated they did not obtain a permit to make changes to home. which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will submit a city permit for changes done to the home along with and updated facility sketch. LPA advised a new fire clearance will be required due to changes.
Based on observation, the licensee did not comply with the section cited above. Licensee completed Mandated Reporter training on 09/04/2019 which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will submit a current Mandated Reporter training certificate to CCLD by POC date.
Based on record review, the licensee did not comply with the section cited above. Kayleigh George does not have immunization against pertussis, measles and influenza on file which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will submit immunizations for Kayleigh George to CCLD by POC date.
