Based on observation, the licensee did not comply with the section cited above, LPA observed a 3A40BC fire extinguisher that was last serviced on October 2023, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit proof of new fire extinguisher and/or serviced current fire extinguisher by Plan of Correction due date, June 3, 2025.

Based on record review, the licensee did not comply with the section cited above, LPAs observed that licensee and husband did not have 1st aid/CPR certificate proof, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit proof of updated 1st aid/CPR by Plan of Correction due date, June 3, 2025.

Based on record review, the licensee did not comply with the section cited above, C1, C5 missing MMR vaccine proof, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit MMR vaccination proof for C1 and C5 by Plan of Correction due date, June 3, 2025.

Based on record review, the licensee did not comply with the section cited above. Child 1 does not have current immunization records which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit immunizations for child 1 to CCLD by POC date.

Based on record review, the licensee did not comply with the section cited above. Husband Dave and daughter Michelle are missing immunization against pertussis and measles which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit immunization against pertussis and measles for Dave and Michelle to CCLD by POC date.

Based on observation, the licensee did not comply with the section cited above. LPAs observed that the home was not clean including floors were dirty and covered in dog hair, kitchen had soiled pots and pans out along with garbage on the counters, outdoor area smelled like dog urine and toys and outdoor equipment were dusty and dirty. There is a screen attached to the door that leads to the backyard that is frayed and unsafe for children which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit photos of cleaned home to CCLD by POC date.

Based on record review, the licensee did not comply with the section cited above. Child 8 is missing LIC700 which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit LIC700 for child 8 to CCLD by POC date.

Based on record review, the licensee did not comply with the section cited above. Child 1, 8 and 9 immunization records need to be updated which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit immunization for child 1, 8 and 9 to CCLD by POC date.

Based on interview, the licensee did not comply with the section cited above. Licensee did not have a current fire and disaster drill log which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will conduct a fire drill and submit documentation to CCLD by POC date.

Based on interview, the licensee did not comply with the section cited above. Licensee is not keeping a safe sleep log for infants 24 months and under which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit a letter stating she understands the regulations and will begin keeping a log for all infants in care.

Based on record review, the licensee did not comply with the section cited above. Husband Dave has not completed Mandated Reporter training which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit a completed training certificate for Dave Jewett to CCLD by POC date.
